Amendment 2- Correct verbiage in section M- Evaluation Factor for Award. 52.212-2 (DEV) Evaluation Factors : "Technical, Past Performance and Price. Evaluation factors other than price when combined are significantly more important than price." M. 03.01 Technical Criteria ( Accreditation status of the proposed contract facility(ies);) to 4. 1. Level of diversity of services proposed; 2. Driving distance and conditions to and from the community-based providers; 3. Enhancements to the basic contract requirements proposed by the offeror which will facilitate institution’s ability to conform to the BOP’s stated mission; 4. Accreditation status of the proposed contract facility(ies); Amendment 1- Fix clerical error on attachment 1. Updated Optometrist to 2d1. Attachment updated. This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with part 12. This announcement constitutes the only solicitation. Offers are being requested and a separate written solicitation will not be issued. Solicitation number 15BCMS26R51700001 is issued as a Request for proposal (RFP) for Comprehensive Medical Services. This acquisition is not set-aside for small business concerns. This solicitation incorporates provisions and clauses by reference. The full text of provisions and clauses may be accessed electronically at www.acquisition.gov. (ii) Please see Section B in the attached solicitation for details regarding Line-Items. (iii) No brand name requirement for the items that are to be acquired. (iv) Services rendered will be provided at FDC Houston, located in Houston, Texas. The date and times will be determined after award. Please see attached solicitation for details. (v) See solicitation for list of provisions. (vi) See solicitation for list of clauses.
